Required Forms for Nannies and Domestic Staff:
Placement Application – This is the first step in hiring process. Please complete the form and fax or email back to us.
Authorization for Release of Information - Before the placement is completed, we conduct the background check. Your signature on the form allows us to perform the background search. Please sign the form and send back to to us via fax or email.
Physical Examination Verification (required for all nanny positions) – This form must be completed by your Physician. It is required by the state of Illinois that all domestic childcare providers have this form up to date and on file in our office before their first day of work.
